Clinical Observation of Salmonellosis and Panperitonitis due to terminal Ileal Perforation



Abstract
Recently the incidence of salmonellosis has a tendency to increase during last decade in Korea.
The mortality was decreased remarkably, following highly development of antibiotics with chloram
phenicol. In salmonellosis, surgical problems such as intestinal bleeding or intestinal perforations were
encountered in 51 cases among 248 cases which authors experienced from Jan. 1967. to Dec. 1970 at
Korea University College Hospital.
We made following results from observation of 248 salmonellosis.
1) In 248 cases of patients during last 4 years, ther were 107 cases (43.1%) in 1970, 52 cases
(21.0%) in 1969, 48 cases (19.3%) in 1967, 41 cases (16.5%) in 1968, in order.
2) The most common seasonal incidence was in summer and autumn, 34 cases(13.7%) in July, 31
cases (12.4%) in August, 30 caes (12.0%) in Sep. and Nov. in order.
3) The ratio of male to female was 1.1 : 1 and peak of age incidence camp in 20 to 40 age group.
4) The duration to admission from onset was most commonly between 1 week and 2 weeks (43.6%).
5) The duration of hospitalization was most commonly from 1 week to 2 weeks (43.6%).
6) The most common clinical symptoms were fever(85%), chilling(79.4%), joint pain(43.5%),
and headach(37.9%), in order.
7) The most frequent physical finding were hepatomegaly(28.9%), abdominal tenderness(23.7%).

8) At first medical examination, the most common range of febrile attack was between 38 degree
C. 39 degree C.
9) In the hematologic finding at admission, the leucopenic change was in 39.1% and the number of
normal leucocyte value was in 44.7%.
10) The results of blood culture and stool culture were positive in 38.8% and 30.7% respecively.
11) The activity of SGOT and SGPT were increased above 50 S-F units 45 cases and 30 cases
respectively.
12) The resul of Widal test was positive (above 320¡¿) in 42.3%.
13) In 52.8% of febrile cases, the fever was dropped within 5 days after application of chloramph
enicol and in 83.4% of fever was dropped within 10 days.
14) The complication of Typhoid fever were intestinal hemorhage (melena) in 14.5% and intestinal
perforation in 6.0%.
15) In 66.6% of perforated cases, the perforation was developed from 10 days to 20 days after onset.
16) In intestinal(ileum) perforation, the subdiaphragmatic free gas was revealed in 60.6% on X-ray
findings.
17) In perforated cases, the simple closure and resection and end to end anastomosis were applied,
and operative complications were wound infection in 53.3% and fistula formation in 20.0%.
